About Us
Unite Oklahoma is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Oklahoma is sponsored by the Oklahoma Health Care Authority, Mercy Health, and CREOKS Health Services. The network is supported by a Oklahoma-based Unite Us team focused on community engagement, network health and optimization, and customer success.
Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.
Community Roots
Unite Oklahoma builds upon work that began with the Community Service Council in 2017 with a Unite Us-powered network focused on serving veterans experiencing homelessness or imminent eviction and connecting them to community services and resources in Northeast Oklahoma. In June 2018, the network expanded with new providers and a focus on the larger homeless population. Beginning in October 2020, the network started the expansion across all of Oklahoma to serve the broader population seeking assistance.
Unite Oklahoma is the network that supports Governor Kevin Stitt’s “Front Porch Initiative” which makes it easier for Oklahomans to get connected to health and human services, connecting government agencies with community-based organizations. In Oklahoma County, the network is being launched in partnership with the Employment and Training Alliance of Central Oklahoma.
Unite Oklahoma serves all residents in:
Canadian, Carter, Cherokee, Creek, Comanche, Le Flore, McClain, Muskogee, Oklahoma, Okmulgee, Osage, Pontotoc, Rogers, Tulsa, Texas, Wagoner, and Washington Counties.

Network partners are now able to send and receive secure, electronic referrals in Arizona, Arkansas, Colorado, Illinois, Iowa, Kansas, Louisiana, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada, North Dakota, Oklahoma, South Dakota, Texas, Wisconsin, and Wyoming through the Unite Us Platform, which ensures communities and individuals can seamlessly access much-needed resources and services across state lines.
